Locate an Adult Day Care Program in your area. Long-Term Care Ombudsman Program (Call an Ombudsman if an inpatient is having a problem with their facility) Adult Day Care –Report abuse in program; licensing issues Adult Protective Services Concern that a disabled adult 18+ or elder person 65+ living in the community, not in a Long-Term Care Facility, is being abused, neglected or exploited. Community Care Services Program (CCSP)- assists older and/or functionally disabled consumers, their families, and caregivers in achieving safe, self-reliant lives. Participants must meet the same medical, functional and financial eligibility criteria as for placement in a nursing facility under Medicaid and be approved by a physician.
